Our mission is rooted in loyalty—loyalty to the individuals we serve, to the caregivers who support their loved ones, and to the community we proudly stand beside. We believe that everyone deserves access to reliable health information, compassionate support, and resources that promote optimal functioning and quality of life.

Who We Serve
Loyalty Community Outreach and Home Health Care is dedicated to serving individuals and families across the Waller and Surrounding Counties who face barriers to health awareness, health access, and essential support services. Our primary focus is on:
1. The aging population
2. Individuals living with physical, intellectual, or developmental disabilities
3. Individuals and families with limited income or financial hardship
4. Family caregivers and informed caregivers who provide support to loved ones
5. Individuals managing chronic health conditions
While these populations represent our primary focus, we believe health education programs and wellness resources should be available to everyone. Therefore adults 18 and older throughout the community will not be excluded from participating in our health fairs, wellness initiatives, preventative care resources or guidance services.
We are committed to promoting health and wellness for all while prioritizing support for those with the greatest need.
