Asthma Care Coordination

$40.00
Asthma Care Coordination is our way of helping you or your child breathe easier, with fewer emergencies and more good days. Instead of leaving you to juggle inhalers, triggers, and doctor advice alone, we walk beside you to build a clear, step-by-step asthma plan that makes sense for your home and daily routine.

We start by reviewing your current symptoms, medications, and any past hospital or ER visits. Then we work with you to identify triggers such as dust, smoke, pets, or seasonal changes, and make a simple plan for reducing them. You will learn what each inhaler is for, how to use it correctly, and how to recognize when mild symptoms are turning into something more serious so you can act early.

As part of coordination, we can help you track peak flow readings if you use a meter, organize your medications, and prepare an easy-to-read action plan to share with family members, schools, or caregivers. This makes sure everyone who cares for you knows exactly what to do during an asthma flare, which often prevents late-night emergencies.

We also help you get more from your medical appointments by teaching you what to report and what questions to ask. For families in rural areas, we can combine in-person visits with phone or telehealth check-ins so support is not lost between clinic visits. Optional variations of Asthma Care Coordination include more child-focused education, added home-environment walk-throughs, or extra teaching on exercise and sports participation.

The benefit for you is simple but powerful: fewer missed work or school days, less fear around each cough or wheeze, and a clear sense that your asthma is being managed, not managing you.
